Community Memorial Hospital

Community Memorial Hospital (CMH), a critical access facility, is one of the smallest full-service hospitals in Ohio. It is a team of caring professionals committed to serving the community by providing quality primary healthcare services and promoting healthful living. Areas of specialties include respiratory care, laboratory, imaging services, diabetes education, comprehensive rehabilitation and wellness programs. In addition, the hospital provides pre-employment physicals, functional capacity evaluations and drug screens. Educational and injury prevention services are also offered, such as ergonomic programs, back safety education, and annual health fairs.

CMH has been serving the citizens of Northwest Ohio and adjoining Indiana communities. It is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
